Before Your Visit
- Email all previous medical & vaccination records to putnamvet@aol.com
- Have a list of current medications (names and dosages)
- Obtain pet insurance information (if applicable)
- Completed new client registration forms (can also be completed in clinic upon arrival)
What to Expect
- Check-in: Meet our friendly reception team and weigh your pet.
- Exam: A thorough head-to-tail physical evaluation by our veterinarians.
- Diagnostics: Any necessary tests or imaging discussed with you.
- Checkout: Review treatment plans, medications, and schedule follow-ups.
